Orthostatic hypotension in the Holmes-Adie syndrome.

چکیده
Two patients who presented with symptoms due to orthostatic hypotension were found on examination to have the Holmes-Adie syndrome. Physiological investigation suggested that they both had an afferent block from baroreceptors in contrast to the efferent autonomic block found in most other cases of idiopathic orthostatic hypotension, including the cases of multisystem disease, now often called the Shy-Drager syndrome.

Asymptomatic Severe Vagal and Sympathetic Cardiac Denervation in Holmes-Adie's Syndrome
A 40-year-old woman was found to have bilateral Adie's pupils and generalized muscle stretch areflexia. She did not have orthostatic hypotension but, in an ECG strip in the office, she appeared to have an almost fixed heart rate. We thus studied the heart rate variability (HRV) and the systolic blood pressure variability (SBPV) in supine and standing position and also during rhythmic breathing....
متن کاملNeurocirculatory abnormalities in Parkinson disease with orthostatic hypotension: independence from levodopa treatment.
Patients with Parkinson disease often have orthostatic hypotension. Neurocirculatory abnormalities underlying orthostatic hypotension might reflect levodopa treatment. Sixty-six Parkinson disease patients (36 with orthostatic hypotension, 15 off and 21 on levodopa; 30 without orthostatic hypotension) had tests of reflexive cardiovagal gain (decrease in interbeat interval per unit decrease in sy...
متن کاملA case of Shy, Drager Syndrome
SUMMARY Shy, Drager syndrome is multi system degeneration of the central nervous system along with pandysautonomia. It begins after age 50 and affects men twice as women. The cardinal manifestation are due to basal ganglia and orthostatic hypotension. In spite of extrapyramidal involvement the response is poor to conventional anti-Parkinsonian medications and if this is the case in a middle ag...
متن کاملA hypothetical immunemediated unifying mechanism for the Holmes-Adie syndrome.
The Holmes-Adie syndrome consists of pupillotonia, arreflexia and autonomic dysfunction. Some explanations for these different symptoms have been attempted, centred upon neuropathological and electrophysiological findings. A hypothetical immunemediated mechanism, as in the Guillain Barré syndrome, is presented in this paper for explaining the three chief symptoms of the syndrome.
